Notes

1. This form should be used to provide details required for the banns to be called.

2. Banns must be called on three Sundays before the wedding can take place. Please allow enough time (preferably at least a month) for this to take place.

3. Banns must be called in the parish church of the parish / parishes where each of you is resident and where you are to be married. Please include the dedication and location of the church, for example: St Andrew's, Hampton or All Saints, Evesham. Please write 'Unsure' if you not certain about the details of the parish church.

4. All the fields in the form must be completed accurately.

5. Please ensure your telephone and email address are correct so we can contact you.

6. The fee for the publication of banns is £34 and the Banns Certificate is £17 (required if the wedding is to take place outside the Benefice). (The fees are set by the Church of England and are subject to change.)

7. Privacy: The personal details you provide will be entered in the Banns Register (the permanent record of banns called in the church). This form will be deleted in line with our Data Retention Policy.
